The following patient/medication safety VIC Teaching Modules were created by students and faculty members at the Leslie Dan Faculty of Pharmacy (LDFP), University of Toronto. The authors would like to acknowledge the 2017 LDFP Teaching Innovation Award that supported the development of the VIC Teaching Modules, pharmacy students from the LDFP and the School of Pharmacy, University of Waterloo, in the development of the teaching modules, and all respondents who participated in our pilot testing and online evaluation. In addition, the administrative and in-kind support from the Institute for Safe Medication Practices Canada for the 2017 LDFP Teaching Innovation Award application was greatly appreciated by the development team.
